I hate to say this but I really don't think this girl is going to get much better and stay better.
You need to start thinking about what you are going to do to help her if she continues to get worse.

She may have Mareks disease. She looks like she is weak on one side and has wry neck, both symptoms of Mareks disease. Contact your state vet tomorrow to obtain a necropsy and test for Mareks at this link:

A person on a different thread had a hen with similar symptoms-wound up being lead poisoning from old paint chips in the chicken yard where they free range.

I've been reading about Merek's. It is a lot to read. Thank you. I hope it is not this. I have 12 new chicks in my garage that were hatched on 4-25-18! Will they be affected? They were vaccinated at cackle hatchery. ???

The chicks may be fine.
I wouldn't worry too much about them right now.

I put her out in the yard, she walked around like a normal hen. Then she sat down and stayed there. I took her back to the coop with the other two.

Did you actually read the whole thing on mareks disease?
It says not to keep the diseased hen near any other chickens
