chick pipped at the wrong end... what do i do?

Make the hole a bit bigger and see if it is breathing.

I've had two or three myself lately hatch out the wrong end, two of which I helped with their zipping once their blood vessels inactivated and their yolk absorbed. They did fine, as did another which got out on its own, as I'd given it and the incubator to a friend to hatch out and told them not to intervene in the hatch unless completely necessary.
 
It's probably just where the yolk went in -they look very bulgy and weird for the first day or so- and then the egg gunk and membrane residues from the egg. Most of mine are like that when they hatch out.
