Has anyone else.had this happen to one of their hens? Notice the middle toe.It is completely 180° and toe nail sticks up. Doesnt seem to bother her walking. Just noticed yesterday when I picked her up

Not sure what happened there. How long have you had her?
Could be from injury or a genetic/developmental defect. Since the nail is sticking up, I would just watch it to make sure it doesn't get too long. You can trim it up a little as needed. https://www.backyardchickens.com/articles/beak-and-claw-trimming.74570/

It's hard to see in the photos, but are her scales raised too? I would give the feet a good inspection for raised scales and look at the bottom of the feet while you're at it for any signs of scabs or swelling.
