Cold weather chickens, how do i know if the moisture is too high and temp too low??

I've done something different with my coop this year, yes it is slightly insulated, but their combs are still getting frost bite.
Last year, first winter, I kept the coup sealed up except the door to the run, fully insulated. It didn't drop below 25 often, IN the coup. A couple had slightly frost bitten combs.
This year I added on to the coup making it a 8x8, exterior. I have it insulated well, except the ceiling of the addition. There I loosely put a 4x8 sheet of extruded up there, with air flow through still. I also keep the north window, cracked, solar run cooling fan for summer, unblocked. The west window I leave about 4" open, it's under a large overhang and can't get rain or snow in. When it dropped to single digits and windy, I close it for a day or until the storm passes. It is supposed to be sub zero the next few nights. I added a heat lamp running 2hrs at dawn and 3 around dusk.
What am I doing wrong? or right!
 
How many birds are in the coop?
Can you post pictures of the coop and where the ventilation is?
You need to strive for dry, not warm.
I can go get pics. 6 hens, 1 rooster. One window, low mid, other mid high, 6" fan is high. There are 3 ridge vents about 2" x6".
Maybe if I open the windows a bit more then.
