cranial development problem **really gross graphic pics**

I recently had a very disappointing hatch I had over a dozen eggs that made it to lockdown but had not hatched at day 23, with no peeping or pecking. I decided to open them to see if I could tell the problem.. Most of them had internally pipped and died. Then I found motion, It was not even internally pipped. I pulled back the membranes around the beak and the beak looked wrong: the bottom was soft and longer than the top, and slightly crossed . No way this one ever could have gotten out. So I went ahead and pulled back the membranes and discovered that the brain was on the outside of the head. The area behind it was swollen with fluid. There were two chicks like this. I am wondering if this is an incubating issue, and if so, at what point in the incubation would this occur, and what type of incubation issue would cause this. I am posting thumbnails, don't click if you don't want to see the full size pics. First pic is right side, second is left, third is holein cranium, fourth is cross beak.
