Curled toes and limping

I have a hen who I just noticed was limping and then realized her toes are curled and kind of gross looking (for lack of better wording?)
I don’t know what to do for her. Do I need to separate her from the others?
She is less than a year old. She eats Dumor layer feed and has access to grit and oyster shells.
