Dead chick in shell question

I finally decided to pip these guys looks like they were gone on about day 18 or 19 one of the 3 had green stuff around the head and being me I have to investigate further the eggsack looked like it was deflated could this chick have poked it's sac? (I was VERY VERY careful and could see the green through the membrane) If so was it human error (position, etc) or too sharp chick toenails?

I believe what you are seeing is just evidence of them being dead a few days.

Day 18/19 they do normally pip up through the inner membrane into the air sac.

out of the 2 that were fully formed they both has egg sacs one looked full like an inflated ballon other was flat I was trying to figure it out why they looked so different neither one had pipped membrane yet
