Denagard Dose for Chicks AND Hens

I have 5 week old chicks that need some Denagard and keep hearing different dosage information. What dosage in cc/PER GALLON should I give my chicks and which for my hens, both for treatment purposes?

Add some sugar to the denagard treated water - it is extremely bitter and can keep some birds from drinking adequately.

I gave everyone 16cc per gal with a cup o' sugar and no one seems to mind! Thanks you guys
