Dirty Chicken Butts

Why do some chickens have horrible nasty butts? It doesn't matter what you do. They stay nasty. Am I doing something wrong? Should I change their diet? Any advise would be helpful. Only about an 1/8Th of my chickens have nasty butts. It is still not ideal.

If you don't worm regularly you may want to do a fecal check.
I have a hen who often gets a messy butt, and I find it helps to cook her up some oatmeal, and also to give her some plain yogurt.
 
I feed them layer pellets from the co-op. They also free range. I do give her raw oat meal as a treat. Would I need to buy special yogurt or just any kind? Thanks for the advice.
