Well, the most annoying autodialers would just start with my area code, get the list of assigned prefixes in the area and work down the list, starting with 0000 and ending with 9999 anyway. It's easy to find the assigned prefixes - here's the list for my area code:

It tells you the cities prefixes are assigned to, which prefixes are cell phone, etc.
In theory, telemarketeers are required to subscribe to the DNC list and remove numbers found on it. There is a ton of latitude in that, though, in how often the telemarketeers have to update to the new lists and how they purge numbers off their lists - and if you've ever done business with and provided a telephone number to a subsidiary of a subsidiary of an allied company, they have your number
