When I was younger it always annoyed me when a man opened a door for me, seemed like he thought I couldn't do it myself (I'm from backwoods NC where that is still pretty common)
As I have gotten older, I recognize it as manners, courtesy, and a show of respect.
I've moved to VA and I can really tell the difference in the manners from here to there.
That being said, my DH and I open doors for each other equally. Whoever gets to it first will hold the door open for the person following as a courtesy. This is done for strangers also.
If I go in a door first and someone is coming in behind me, I hold it open for them so they don't have the unpleasant and rather rude experience of it closing in their face. I also try to remember to say Thank you, regardless of who does it.
