Are you talking about Silvadene? The generic is silver sulfadiazine, and it's wonderful stuff. Just a small amount on the wound daily works very well.

Part of why the saline spray (or even just rinsing with clean water) helps---according to the vet---is that products like creams can linger on the surface and build up, so that when you add new product it's not actually making contact with the actual injury.
My routine with Candy was spraying her injury with the saline canister, then patting her dry with a towel, then applying the silver cream. Once the injury crusted over---no longer wet or goopy--I stopped doing the saline rinses and just would apply the cream every day or every other day.
If they would give you antibiotics that would be ideal, but I realize that might not be possible. You could also ask if they'd be willing to do a "telehealth" appointment with you.
It really sounds like you're doing a great job and she's moving in a good direction.
