Hard for me to tell if that is urates or egg white. She may have had an egg break inside. Keep up the calcium and soaks, hopefully she will continue to expel any egg material, if that is what this is.

Would it be helpful if I posted what came out of her?

Okay, I was thinking she could be. Is she bloated a lot? Compare her to another hen.

She likely has a reproductive issue, and if she hasn't been laying that's another sign of it. The tail pointing downward is also a sign.
It may be hard to know exactly what reproductive issue she could have. It could be Salpingitis (if she has that she will be expelling lash eggs, which is basically pus), Egg Yolk Peritonitis, Internal Laying, ect...
Bloating of the abdomen can also be a sign of ascites, which is from organ failure. The organ will leak fluid and it accumulates in the abdomen. I think its reproductive though due to the tail pointing down.
Unfortunately in most cases there's not much you can do. But you could give her some antibiotics and see if it helps. I believe the suggested one for reproductive issues is Enrofloxacin.

Yes she has always seemed more puffy than the other chickens in her breed but I always just thought she was fatter. Would giving her calcium help for reproductive issues or probably not? The weird thing is it just randomly started. Can that happen with age? She has acted like this before but has always recovered.

Yes, calcium could absolutely help. It is always recommended. You'll want to give Calcium Citrate +D3, with at least 400mg of calcium. Give it once a day, for about 5 days and see if it helps.

Yes she is! She is about 3 this year. Its happened maybe 1-2 times. let me see if I can find a picture of her before she got sick.

Poor thing! She doesn't look like she's feeling well. The comb color difference and floppiness does mean she's sick.
I would start giving her high protein food such as meat, fish, and eggs. Also give her some water. I'm going to guess she probably doesn't feel liking eating and drinking much.
If you have anything like Nutri Drench I would start giving that.
If you're up to it, and want to put some effort into her, I would say antibiotics could be worth a shot.

here she was when she was feeling well. I don't think her hind end was this puffy before.
