The hole is covered by fatty tissue which is why it makes a puff when the feathers grow in. The larger the puff the bigger the hole. Most cresteds don't have issues but it is something to be aware of. If you ever do breed them to another crested 25% will die before hatching and I believe 50% will be crested and 25% will not be crested but will carry the gene. You must have a male crested to get crested offspring. There is quite a bit that goes into breeding crested and it is not for the faint of heart. Some will have medical issues and some will not hatch. It is not worth it to me to breed something that is known to cause problems. They are super cute and I had a male crested. He was quite healthy and did just fine. Be careful with the females though if you ever add males due to them having that hole they can get injured during mating since males grab the heads of females and push them under water.
