IMPORT HEALTH REQUIREMENTS OF CANADAFOR POULTRY FROM THE UNITED STATES OF AMERICA
Poultry must be accompanied by a U.S. Origin Health Certificate (VS Form 17-6) issued by a State Veterinarian or a veterinarian accredited in the State from which the exportation originated and endorsed by a Veterinary Services veterinarian. The certificate shall contain the name and address of both the consignor and the consignee. Additional information shall include:
CERTIFICATION STATEMENT FOR POULTRY (Chickens, Turkeys, and Game Birds)
1. To the best of the knowledge and belief of the issuing veterinarian, the poultry are free from communicable diseases and have not been exposed to avian pneumoencephalitis (Newcastle disease), fowl plague, fowl typhoid, pullorum disease, or ornithosis. (Checking columns A or B under Item 12 on VS Form 17-6, "Certificate for Poultry and Hatching Eggs for Export," will meet this requirement.)
2. The flock of origin is free from pullorum disease and fowl typhoid under the USDA National Poultry Improvement Plan (NPIP). (Providing the NPIP approval number in column 9 on VS Form 17-6 will meet this requirement.)

The flock of origin (all birds over 4 weeks of age) has been tested negative for pullorum disease and fowl typhoid within 12 months prior to export. All poultry being exported must have been resident in the flock of origin at the time of the test or been added either through natural increase or through additions from a flock(s) having the same status.
(This certification must be provided under Item 13 on VS 17-6, "Certificate for Poultry or Hatching Eggs for Export.")
NOTE: All poultry and game birds must be identified. This can be done by such means as leg or wing bands.
POULTRY FOR EXHIBITION (Chickens, Turkeys, and Game Birds)
All poultry exported for exhibition purposes which are not under the USDA, NPIP, must be shown to be individually negative to tests for pullorum disease and fowl typhoid within 30 days prior to export and must be returned to the United States within 30 days from the date of entry into Canada. Poultry under 4 weeks of age do not require an individual test. These poultry must meet the requirements of certification statement No. 1 on page 1 of this attachment and be certified on VS Form 17-6, "Certificate of Poultry and Hatching Eggs for Export."

I found it, it is pretty simple for a NPIP flock to do, I am sure there is money involved though
