Egg bound? worms?

4 1/2 year old hen less active, waddling, and just sitting down away from other hens. Picked her up and her abdomen is grossly swollen. She has had chronic diarrhea for a while, nasty butt feathers.

Soaked her in Epsom salts, cleaned off all the crusty poop. Her color is good. Her vent is small and dry. I don’t think she has been laying much or at all this past summer. Not sure what could end going on, if she’s not laying, how could she be egg bound? Don’t see worms in feces.

It's unlikely she is.
Her symptoms sound like ascites which, in an older layer, can be caused by a tumor in the reproductive system that causes liver failure and the liver starts to leak fluid into the abdominal cavity.
You can relieve some of the pressure by draining her abdomen and give her some more comfort but there is no cure.
Or you could euthanize her.
