egg problem

one of my hens has started to lay a poopy egg. poop on the egg not in the nest and sometimes I find the egg out in the run somewhere.
I have done a search and can't find a solution. This has been going on for a week or more.
She is a year old.
Started adding ACV in the water last two days.

to poopy to clean so I just give it back to the chickens

I have gotten a few eggs the same way over the last few weeks. I also just started adding acv to the water, to try to up production.
