Does anybody know what might have gone wrong?
I candled on day 10 and everything looked good. Veins movement etc.
Day 11 and 12 I candled and the veins had all disappeared and there was no movement at all.
I felt certain it was dead but now I’m worried I misjudged

It no longer moved independently, just moved when I moved the egg. It didn’t seem alive and all the veins had disappeared
 
Overheating is highly likely.

Do you have at least one calibrated thermometer in your incubator?
 
You cannot trust a built-in thermometer on pretty much any incubator.
You must purchase a separate thermometer to calibrate it always.
I keep three or four thermometers in my incubator at all times so I can tell at one glance what the temperature is and all areas of the incubator.

It is very common to have incubators that have hot spots in them... Hot spots kill quicker than a cool spot.

That is very true one side of mine runs warner so I move my eggs around every time I turn them that way nobody is staying in the hot spots to long
I also keep 3 calibrated temp readers inside as I don’t go by my incubators readings
