Empty egg shell? What does it mean?

Just let my girls out of their pen to range and one of the Golden Comets which has been laying for about a year stayed inside. Unusual behavior so I went to check it out. She had something hanging from where the egg comes out and about the time I decided I would have to pull it out, she ran off and after a few feet, it fell out on its own. It has the consistency or shape of a morrell mushroom with some other tissue attached. It looks like it could have been a shell that never got filled? Is this something anyone has seen or heard of? Does anyone know if this is a sign of illness or just something that happens or is it the world's newest mystery?
