I have a running list from the last 3 years, but not for the preceding 3 years.
Not going to format it like yours tho, sorry,

What diversity some flock histories can have!
Any hatched from your flock? What is average age of retirement? What is cause?

These are questions of interest to me. I hypothesize you will see two major types of flock history. First will be where the majority of all birds are outsourced from commercial hatchery and leave few if any descendants. The second will involve replacements breed on site and will be the source for some chickens used by other type of flock manager.
 
I have not yet had chicks from the flock. Since I'm pretty new. But after much research, I will be giving it a try soon!

The average life span for a chicken about 8-10 years
Unless some kind of predator Fox, Coyote, Hawk, etc. :(
