Hi all, no surgery for me. They want to do that ablation to kill the nerves, go see a different dr for my hips and some type of therapy that wont stress my heart.

oh and he wants me to go see a different vascular surgeon for my legs

1.	Chronic bilateral low back pain, unspecified whether sciatica present
2.	DDD (degenerative disc disease), lumbar
3.	Spondylosis of lumbar joint

Plan:

Ms. Churchill presents with back pain and bilateral hip pain as well as extremity pain. She has a complex medical history and suspect that at least some of her issues are vascular related. She has been evaluated before but it was noted to not be severe enough for surgery. Suspect that this is worsened. Do not see any surgical spine indication at this time.
Will refer to vascular surgery at Kings daughters Hospital in Madison
Will refer to Dr. Schulte at Saint Elizabeth spine center to discuss radiofrequency ablation for back pain
Dr. Auer for bilateral hip osteoarthritis as this is her primary concern now
Start physical therapy and home exercise program in the meantime
Follow-up in the office as needed. Call if she has any concerns at all or condition changes.

Had to have the well service people out today to put in a new and more powerful pump. I put the old one in about ten or twelve years ago. It was about due. Pump, wire, control box and labor was a little over 1600.00 dollars. Less than I would have figured.
