so although Myth busters couldnt recreate it it is 100% possible. mech. failure is the main cause. Ice builds up under the plains in flight causing cracks in the lines or murphys law causes the safety valve to fail with the biggest hunk of blue ice wound be about 3ft.

not the ice causing the failure but the failure causing the ice. My bad.

yikes! It sounds completely plausible, but has there ever been real evidence of it happening? How'd you like a chunk to fall through your roof?
