You might want to talk to them about what they are really looking for... and if they do find something they are looking for, what will they want to do about it? And what are the chances of successful treatment? Will it make a significant difference in his quality of life? Since Tom is not interested in any more big procedures, you have to consider whether this test is worth it. At the very least, it's going hurt like heck for a time. Maybe best to let the man just enjoy his fish.
Anyway, it's worth having a conversation with his dr... just so you know what it's all about.
