GRAPHIC PICTURE!! malformed beak on deceased duck??!

i'm looking for answers. at this point I'm referring to this duckling as a specimen, just because its so odd...
This specimen has a malformed beak (top) and legs that turn in at the feet. The specimen doesn't have an egg tooth, it had a white spot where the egg tooth should be.
anyone have ideas on what happened here? humidity and temp were monitored very closely during this hatch.
