GRAPHIC PICTURE!! malformed beak on deceased duck??!

i'm looking for answers. at this point I'm referring to this duckling as a specimen, just because its so odd...
This specimen has a malformed beak (top) and legs that turn in at the feet. The specimen doesn't have an egg tooth, it had a white spot where the egg tooth should be.
anyone have ideas on what happened here? humidity and temp were monitored very closely during this hatch.

Are you using an app to monitor constant temps?

Okay, there was an issue with the turner at one point, that could be it.
