Half a bird

Incubating problems. My grandma was hatching some duck, pheasant, goose, and chicken eggs out and nothing hatched. They waited a week later to open them just to check to see if there was any bird in the eggs. Once they opened the shell there was only have the bird and the rest never developed. Does anyone know what can be causing this. They have hatched many birds out of this incubator before and never had this problem. Please help. Thank you.

Usually, if the laying stock is healthy, developmental problems result from innaccurate temps, (have their thermometers been checked lately for accuracy?) and inadequate egg turning can also contribute to development problems.
