Do you mean this was hatched by a broody or inside an incubator that way.. and without assistance?!
If in an incubator.. On day 21? Very likely a temperature anomaly on a cert day(s).
I personally would cull instead of letting a chick die slowly. But it's ALWAYS a difficult choice.
According to the following link.. exposed brain is caused by high incubation temps or low oxygen during DAYS 1-3 after setting.. Page 7 #24 topic for discussion.. Consider bookmarking the link.
Hatchability Problem
Hope this goes a ways toward helping discover what happened.. my guess might be a temp spike with fortunately this being the chick most effected.. Or MAYBE closest to the turner, etc..
