I think a Warren hen is the same as what we call in the USA, ISA brown hen. A production hybrid breed, we have 3. I didn't know about some of their troubles when we first purchased them. Our eldest of the three is 26 months and is now laying 4 a week instead of 6. This may help her live longer. Our other 2 at 16 months are laying 5-6 per week. I watch the older more, now that I know this. Hopefully epsom baths will help.

Curious, I thought it was 250mg 2x daily = 500mg. You say it should be 1000mg daily?
750mg is the total with 3 x 250mg doses.
The dose range for Amoxicillin is 125-250mg/Kg PO every 8-12 hrs. Your average hen weighs 2.27Kg (5#).
I have also used it just twice daily. But when a hen is obviously very sick I go to a higher dose.
The OP's hen is showing signs of illness and when they get that far they're already pretty bad.

Sorry about your hen. It’s tough to make the call to cull, but it’s harder to see them suffering.
