HELP MY CHICKEN HAS BLACK COMB AND IS DIZZY

he is dizzy about 24/7 he sleeps with me I provide him out side play and is falling all the time starting 3-4 months ago help me I will tell my dad to put him to sleep if he don't get better.thank you
