Help! Single hatched chick

Ok, he might be dead. Sorry :( keep him in the same set up in case. Don’t lose hope but prepare yourself for a loss.
I’m keeping him on the heat pad but I’m almost positive he’s dead. I feel bad obviously because it’s a loss but also because this is the second time my hen has had an unsuccessful hatch. The first time she ever laid on eggs they both never hatched and I’m sure the rainy, cold weather contributed to that. I eventually had to remove the eggs because she wasn’t coming down to eat and drink and that was almost 4 weeks.

You might not have done anything wrong. If you didn't notice him move when you originally brought him inside he was probably already gone.
What are the common reasons for them dying so quickly?
