HELP! What went wrong? (WARNING: Graphic pictures)

yes there are 2 of them you are correct. The thinner one by the chick has veins and carries the chicks blood supply untill the yolk is absorbed. that chick was not fully developed and looks like it died about 2-3 days before it should have hatched. If the eggs were in there for the full 21 days it either died a couple days before hatch OR your temps were to low and the chick never really fully developed by day 21.
 
Did the other chicks have unabsorbed yolk? A bacteria that can go through shell can contaminate a whole hatch causing "Omphalitis" or mushy navel. The bacteria is common and can be found sometimes in eggs with manure on the eggs. Too high humidity keeps the navel from closing and the bacteria get in there anytime but you see symptoms late in growth with late hatch die-offs, dying before/at/just after hatching, wet chick. Solution is to mega sanatize your incubator. I had this happen and lost 7 chicks. Next batch I lost 6 and 2 made it. It wasn't until after the 2nd batch I realized the problem and really sanitized with bleach and water. Sorry for your loss of your hatch. It's so frustrating!
