a lot of times it's where a chick has made a LARGE pip .. and then gotten too weak to proceed any further.. so the membrane dries out shrink wrapping the chick .. the more it tries to break through the membrane the weaker it gets because the membrane is too dry and too tough..
when you look at the membrane you can tell right away that it looks thick and dry
I used google and found this ...

So basically his/her head (not her beak) were kinda stuck under the shrink wrapped area. I decided to take (not sharp) tweezers and pull away what he/she really could get through! After that he/she was resting a little bit and then began to zip! Right now she/he is in the brooder box! So glad he/she's okay!

Yorray! I've only hatched once (so far!) and decided that my error shouldn't be the cause of lost chicks, so I help some pip and zip and PT! I read for hours on BYC to learn what to do. I figured it might die if I help, but it would definitely die if I don't help. My logic.

Congrats on the little! Watch it's legs and see if it can walk well; mine didn't walk so well and needed PT to get to food and water.

Yeah I have been watching him/her walk and its getting better little by little. He/she rested most the time and I don't blame him/her! He/she has been in there for at least 10 hours! lol Although, egg #5 is now piping!! So excited! That's 3 out of 6 so far! Although they were all early! Good luck with your chicks!! :)
