Hen has worms what to use for medicine

My hen is about 5 years old. Has been sick acting for awhile. I thought was a respiratory thing the way she was acting and sounding. I got antibiotics Duramycin and she got better. Still lethargic does eat and drink, has dark green real runny poops. She has always had a problem keeping her butt feathers clean. I washed her behind like I have once before and the feathers just fell off. After I was done I dumped the bowl and saw a worm. Small white like 1" piece inch of thin speghetti. I do not know what kind of medicine I should give her. Please any help. She doesn't lay anymore I only have the two left (free range) so they are like my pets. Thanks
 
Pictures of any droppings help. Ideally, it would help to take a few fresh droppings to a local vet for a fecal float to identify any worms, since chickens can have several types. Most feed stores will sell SafeGuard Liquid Goat Wormer or the Equine paste. A full grown hen would get about 1.25 ml or 0.25 ml per pound, given orally. One treatment, then repeated in 10 days would help with roundworm. But I would give it 3 days in a row for most other worms. Capillary worm, more rare, take 5 consecutive doses for best results. Another product, Valbazen, gets all worms with one dose of 0.5-0.75 ml given orally, and repeated in 10 days. Here is a good link with worm pictures:

I couldn't get a picture but I was able to see her bottom and her butt feathers are covered with little worms. I couldn't see them before.
 
Make sure that they are not maggots, which can be very deadly. They can be common in summer heat if there has been an injury or droppings stuck around the vent. I would place her her a warm pan or bucket with soapy water or Epsom salts water, and soak her to get any worms (or fly larvae) off her. Look for any wounds in which the magoots can bore into the body. Apply some plain Neosporin/triple antibiotic ointment to any wounds. Try again with a picture.
 
Yes that's what it is. I've now read some stuff about it but how would you suggest to help her? I washed her bottom too get a better look like little rice. I got what I could off but they are on the inside of her as well. The whole are is about the size of 2 to 3 inch area.
 
She needs to be soaked in warm water with either dish soap, Epsom salts, or betadine, and then make sure to get as many of the maggots off as possible. They should fall off in the water, and afterward, inspect her wound to pull any others off with tweezers. The baths may need repeated for a day or two to get them all. Apply plain neosporin ointment to the wound which may help to smother other maggots. Make sure that she eats well, and give her chopped egg or tuna, plus her usual feed. Water is very important as well. Hopefully she will recover. Do some Googling for "Flystrike in chickens" to read some articles.
 
Ok thanks again. They aren't maggots that much I do know. Is there a medication I could give her to kill them?
 
Flystrike is caused by flies laying eggs of fly larvae on a wound or in the droppings caked on the chicken. Fly larvae are maggots. You said that was what was wrong, but maybe it is not. I would get her in a warm soapy bath right away if that is the problem, because flystrike can kill very quickly. There are many articles with pictures to search. Good luck.
