Hen having troubles pooping, bad balance

My four? five? year old black Australorp hen Knight has been having troubles for a couple months. I've been keeping an eye on her and making sure she's getting food, but she doesn't seem to be better. To begin with, she has troubles pooping, and when she does she lets out a squawk as if it hurts her, and it's usually watery and small and greenish. She also has bad balance and when she tries to peck at something often misses something, though that may also be because she's getting older. I'm pretty sure she's lost some weight as well, as she's lighter when I pick her up. I don't really think it's too urgent, as she's still energetic and curious as always, but I don't feel comfortable leaving her as she is, especially as my parents are working and I'm busy with all my classes and work. I don't know if she's still laying, as we have two other black Australorps in the main coop (and a blind one in her own pen), plus a barred rock that lays very similar eggs. They've also been molting lately, though that's probably unrelated and Knight's problems have been going on for longer. No one else has these problems, though our bantam cochin has been relatively weak for at least a year (she's been on antibiotics before due to swelling on ears and eyes, though the swellings gone down. Still going to make a post about her though). I can't find any injuries and she seems to be breathing fine. She occasionally lays down, though. I've been making sure she's getting lots of water and food (I can't keep doing that because school), and been giving her broken egg shells in case it's a calcium problem.
 
5 yo, she's an old lady. Perhaps it's just the end of her time. Sorry.
