Hen not acting right

I have a small white hen, not sure what breed she is, but she is not acting quite right, I noticed Saturday that she was lagging behind when we would feed them, she would eat but not alot, did see her drinking a good bit though. When we let them out she stayed behind in the coop, I took her out to see what she would did, she went and found her a shade and sat, I tried to give her some yogurt and oatmeal with a little vitamin hidden in it, she ate a little but not enough, I noticed her droppings and they looked normal. I but her in a pen by herself to make sure she was eating and drinking, she is a little but she still just stands around, won't try to run acts like she's in a daze. I noticed this morning that her dropping was yellow, small and looked watery, not really running though. Any ideas? I did notice tape worms last week and gave them wormer, think that is what is wrong.
 
Did you complete the full course of wormer treatment? If so, did you follow up with probiotics? Worming is hard on birds, so follow up to restore gut flora is important. There could be something else going on, but this is a start.

How does she feel? Is her keel bone protruding (sign of weight loss)? Is she a layer? If so, any eggs? Is she in moult? Just some initial questions running thru my mind that may help you get answers.
