1) What type of bird , age and weight (does the chicken seem or feel lighter or thinner than the others.)
Isa Brown hen, about 2 years old, seems a similar weight to the others (couple kilograms?).

2) What is the behavior, exactly.

Noticed she was acting differently to the other hens in the flock yesterday, she didn't come investigate if there was any food being given, but just hanging out by herself.
But after a while, she seemed to act normally again.
Checked on her today and she was just standing apart and keeps trying to rest and close her eyes.
3) How long has the bird been exhibiting symptoms?
1-2 days
4) Are other birds exhibiting the same symptoms?
Not as far as I can tell.

5) Is there any bleeding, injury, broken bones or other sign of trauma.
Not as far as I can tell.

6) What happened, if anything that you know of, that may have caused the situation.
Can't think of anything that could have been the cause.

7) What has the bird been eating and drinking, if at all.
Normally has pellets but has reduced drinking and eating.

8) How does the poop look? Normal? Bloody? Runny? etc.
Unsure, but haven't seen signs of bloody or running poop from her yet.
9) What has been the treatment you have administered so far?
Placed her in a crate apart from the others, in a darker, quieter place.
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
Hopefully get her fully treated.
11) If you have a picture of the wound or condition, please post it. It may help.

12) Describe the housing/bedding in use
Wooden coop shared with 5 other hens, using deep litter method, roams in a run covered with straw.

When was her last egg?
Any lice or mites? Good place to check is around the vent area.
Also check her crop early in the morning and see if its empty.
There's been 5 eggs from 6 chickens the past few days - not quite sure, but she might have laid. I'll keep an eye on that.

Haven't seen any lice or mites, but was dusting them for it a couple weeks ago.

Her crop isn't empty at the moment but she might have eaten a bit. I'll check on whether it's empty tomorrow morning.

Ok.
Any bloating in the abdomen?

I'd check for them.

Sounds good.

Ah maybe egg bound?

Quite protruded but very soft, not hard like an egg.

The lack of feathers in the region has been like that since we got her, we thought dusting her would help new feathers sprout if it was lice/mite related but nothing has grown since.

If you can feel an egg in that lump on her butt (be very careful or you could break it which would likely lead to death) then you need to get some calcium citrate in her right away to help her expel the egg. Time is of the essence. Lacking that you can give her a couple tums but calcium citrate works better from my understanding. I've never had an egg bound hen but I've helped my mom with hers and it can kill them within 48 hours or less.

She has ascites, which is from a reproduction issue.
As of now you can't really cure it, only drain her abdomen of fluid. But it will fill up and need draining again.
