Hen pulling out ALL feathers

I have a hen that is pulling out all her feathers. I thought some predator had got into the pen because there were feathers everywhere. I have been watching to see if she is getting picked on, but it doesn't appear that way. She also lifts one foot, always the same one, often. It doesn't look like molting, unless there is such thing as EXTREME molting. She is eating and drinking and acting normal.
Any ideas?
 
I vote hard molt. But check for external parasites (lice, mites.) My coop and run looked like a chicken exploded last week--but just the BO in hard molt--no parasites found on inspections. 1st hard molt is usually in the first fall after 12 months of age (so approx 18 months old for the previous spring's chickens.)
