Hen Standing Around

I have a hen that I believe is about a year old, possibly two? (I have a mixed flock.) While everyone else free ranges she stands around pretty much all day. She is fairly thin (doesn't look it but feels like it). She sometimes tries to sleep in the nesting box but not always. She eats pellets I throw out in the morning while everyone else eats. I also kept her caged separate a few days and fed her on her own thinking she didn't get enough food. There was always some left over. Her poop has small cecal worm eggs but from what I've read that shouldn't cause much harm? Im not very familiar with it though. Her comb is pale and some of the nails on her feet are stubby and worn down. Not sure if it has anything to do with her, but last year I had to cull a chicken of the same breed because she had gone blind in one eye, but so far her eyes look normal.

Any ideas on what's going on? Ive thought about giving her worming meds but then have to commit to isolating her for 2 weeks should she start laying. I'd love some advice! Thanks in advance!

The worms may be contributing to her poor condition.
If she were mine, I would deworm. You can use Safeguard, dose is 0.23ml per pound of weight given orally once a day for 5 days.

Loss on the end of the toes might be due to Frostbite, hard to know.
