I have a 12-14 week old hen that I noticed last night was walking around with her tail feathers tucked (think dog who knows they did something wrong and tucks tail). When I put all of the girls in the coop and closed the pop door, instead of eating as much as possible before bed like the rest of the girls do, she drank water for about 10 mins and kind of kept her neck hunkered down as if she was trying to get as small as possible.

This morning she still has her tail tucked quite a bit, but not as bad as yesterday, and had a very foamy poop this morning. I checked her crop and it was clear. What might be the diagnosis, and what do I need to do for her?

The other chickens are acting normally, and this chicken is happy to eat any treats or grass that I throw into the sand covered run. She has eaten the same things as the other chicks as well so the issue is not her eating something she should not have (at least not so far as I can tell).

What do you mean by "the crop was clear" - was it flat/empty this morning?
How old is she and when was the last time she laid an egg?
Do you have vet that perform a fecal float on a fresh sample of poop?
Photos of her and the poop may be helpful.

Examine her for lice/mites, any injuries and feel her abdomen for any bloat/swelling.

My first thought is coccidosis. Get some Corid from a Tractor Supply and treat the whole flock. If it powder use 1 1/2 tsp per gallon of water and dump and make a new batch every day. Only offer them that Corid water. If it wont drink, you can syringe it to her every few hours. Do this fast, I lost 1 pullet in a matter of 12 hours from cocci. The others pulled thru by day 3 of the Corid.

Other thought is worms. Worms is not my thing, so I would find out if you can treat for worms at the same time as using the Corid. I think you can and I would do both.

By crop was clear, I mean there was nothing in it this morning, so she was able to pass everything last night. Indicating no sour crop or impacted crop.

She is 12-14 weeks and has not started laying yet.

I will get pictures of her and her poop tonight. When I checked her this morning she did not have any lice or mites that I could see and no bloating or swelling either, but she doesn't love to be picked up so its a little hard to tell the difference between now and normal.

Thanks, I will get some corid and treat the flock!

Here are some pictures of her. There are no more foamy pooos that I can see but there was one poop with a bit of red in it. But still solid.
