Hen's vent has been dirty for some time.

My smallest hen is almost 4 years old, weighs about 3 pounds and has her separate coop from the other hens because she was bullied. She currently sleeps in a repurposed dog house in wood shavings since the space is too small to build a roosting area and seems to thrive in her own space.

The only issue is I've noticed she's the only one who consistently has a dirty vent. She seems fine otherwise and still lays from time to time. I think she was born with a defect so she weighs less than the others and doesn't walk normal.

I wormed the flock last week and give them yogurt about once every two weeks or so. Just wondering what I can give to help with this? TIA.

If she doesnt have a roost to perch on and she lays/sleeps in wood shavings, I'd expect her to have poopy butt.
You're going to have to somehow install a roost above the shavings for her to perch. Then the poop will drop in the wood shavings without her getting a messy butt.
You can also trim back fluff and feathers around her butt area and just below her anus. Do NOT trim them back too much or urates in feces will scald her skin causing redness, loss of fluff and feathers as well as pain.
