I'm not a genetics expert, but to me she looks unwell. Don't take this the wrong way, but she looks like she is not quite right and I would cull her.

Have you tried doing anything special for her, such as giving her vitamins for a long period of time to see if her feather quality improves? Also, try trimming a small bit off of her beak. It will make her profile look a little better.

That curve in her head was from an abcess. It was huge. A few days ago I pulled the rest of the infection out of her head so now there's a little hole closing up. The infection was like rubbery cheese.
 
Well, I doubt she was from bantam stock, I think she's just a genetic mishap, probably from BO stock as you said the other eggs were. Sometimes life takes a weird turn along the embryonic path.
 
I have already trimmed her beak a few days ago. I'm afraid to go any shorter, I don't want to cause her any pain or have her beak start bleeding. Shugie eats pretty well so it's not a problem until next time when I have to trim it.
I haven't given her any vitamins other than whats in her treats and feed. There's vitamins in vegetables and grapes right? She loves grapes.

I thought I had to cull her when that abcess showed up, but I got her all better and we pulled through. I would only cull her if there was something seriously wrong with her. Even though she looks weird, Sugar is quite the sprinter.
 
HOLY COW! I didn't realize this was "The Sugar!" She looks tremendously better. My initial recommendation when you first posted about her abscess was to cull her. I must say I'm glad you didn't. You may very well have a "special" chicken on your hands, but who cares what she looks like? I'm glad you followed your heart regarding Sugar's care.
