I would worm them and start putting food grade DE in with their food. I think once you actually see the worms in the poo it is a really bad case. Do a light wormer first and follow up with a more powerful one in 7 to 10 days. I have heard some chickens with really bad worms do not make it, because their little bodies can't handle it.
