I just found a dead hen

Shes a jumbo. It looks like a broken neck. My cages are only 8" high. While I was watching the cage, one of the other hens stood a hens head and neck, holding her down for a second. Could she the problem? What would you do?

How many birds do you have in your cage and what is the size of your cage?

I'd sit and watch the cage all day...to see what is going on if I were you.

So, you had 5 birds in less than 3 square feet? No wonder you have problems. The minimum amounts of space they should have is 1 square foot/bird. I give mine 2-3/bird.
