I opened an egg passed its due date, What do i do?

One of my hens went broody and we put 6 eggs under her at the same time. 5/6 hatched on time at 21 days and the last one did not. It had been around 5 days after the eggs hatched and I believed that the last one would be dead. I candled the egg and saw no movement and no veins so I cracked the egg open. To my surprise there was a living chick in there with the yolk sac still attached! I've tried researching of what I can do to help the baby survive. I don't have an incubator so I put it under a heating lamp and covered with a damp towl to increase humidity. The baby is still breathing after 5 hours and twitching and moving it's break. I also noticed the yolk sac shrinking in size so I assume the baby is absorbing it. Does anyone know what else I can do to ensure it's survival? Or has anyone else ever experience this?
