I'm back with the same chicken. Loss of appetite, mouth discharge. Frostbitten feet??

Polish can be odd birds sometimes. They can often get pecked by others in their crests which can cause brain damage. I once had a silkie that got pecked and ended up slow, and with obvious brain damage. I had to help her daily. Yours could be a similar situation, otherwise Mareks comes to mind. Mareks cause tumor to grow and push on nerves which than causes paralysis, and various other neurological issues, so if she continues to worsen that would be my thoughts.
The cedar chippings are all gone at this point. Do you think that might've caused it? I must have gotten my information wrong, because I read somewhere that it's only a threat if you don't have enough ventilation. Oh well... She was vaccinated for mereks as a chick. Yes her vision is currently obscured by her huge crest, I've trimmed it before and it seemed to work, but now it grew back or something because I can't see her eyes at all.
 
Yes she does have problems within the flock, she is the bottom of the pecking order, they peck at her sometimes, and she has a small bald spot on the top of her head. I've never seen any of them chase each other from the food

But in short, almost died from malnutrition, used her back to health, been feeding separately for a while. And I've switched from feeding her separately and I've increased the feed intake I'm giving giving the flock and that seemed to work, full crop every night. I suspect the other chickens kicked her out when the food started getting low, and I never witnessed it because I don't stick around that long.

She's very slow, she eats and drinks normally but when she's not doing that she's lying down. For a while she did this "ticking" thing with her head where she would slowly turn her head to one side then it would snap back. Hasn't been doing that since the food incident though.

Here are the pics for the ventilation. I have a metal roof and there's space between the "pinnacle" of the roof (I'm sorry I'm terrible with words, it's the second pic), and there's nothing underneath the bumps on the metal shingles (3rd pic) so air can escape through. And some space between the door and the wall.

If she still has a discharge coming out of her beak, then you need to try to look inside the beak for any obstructions, canker, yellow plaques. Also check to make sure the crop is empty in the morning before she eats/drinks.

I do agree with @oldhenlikesdogs if she is crested, sometimes a peck to the head can cause injury to the point the birds have neurological symptoms.
But you need to figure out if she is still suffering from dehydration, not eating, Coccidiosis, a crop problem or possibly head injury.
With chickens a lot of times it's a process of elimination unless you can have her seen by a vet. Getting a fecal float is a good idea, this will let you know if there is a Coccidiosis overload at the very least.

If she can't see well, then you need to try to trim up her poof more or I saw where someone used hairbands to captured the poof. That may have been on a Silkie. I don't have any Polish so can't tell you how to do that, I just know people trim so the birds can see.

I'm sure @ChickenCanoe will chime in, he is more eloquent than I and can explain ventilation much better, but your coop needs more ventilation than it has if around the metal is all it has.
Does the coop have any windows or vents along the roofline? There needs to be air flow in the coop year round. We tend to want to shut them up in winter, but the opposite is true. The birds need protection from direct wind and drafts, but air needs to be moving to help remove ammonia from droppings and moisture.

No I did not. And okay, thank you for telling me, I will get more ventilation out first thing tomorrow. I bought a vent cover that we use for normal houses, that should work. Last time I checked her crop was completely empty and has been since this afternoon. And I couldn't find any poop that looked recent enough to take a picture of. From what I've seen she hasn't eaten anything all day. Any explanations on the noise? I know I still don't have a video up, so I understand if you have nothing.

I'm so glad this place exists.

I’m chiming in too, to say aside from a “haircut” first thing in the morning, I’d like to see you get some vitamins and nutrition into her and provide steady access to granite grit ASAP.

Human infant polyvisol (no iron formula) or nutridrench,
Work well.
My birds don’t really care for either so I usually direct dose it or soak bread cubes a few drops at a time.

I’ve stained quite a few shirts with vitamin drops, so don’t wear clothes you plan to go to work in!

Hard boiled or Scrambled eggs or something similar could be on the menu tomorrow, after the crest issue is taken care of. Maybe a bit of cheese sprinkled on top... some say chickens don’t handle lactose well but apparently my birds never got that memo.
I scramble eggs in a mixing bowl in the microwave for the pets once or twice a week. Whisk egg(s) cook at Full power 1 minute, whisk again. Cook another minute. Should be done.
Everybody gets a little sprinkle of shredded cheese on top.
All are thankful.

She won’t stand a chance if you can’t get her eating.
Fruit might tempt her as well, although in moderation. You don’t want diarrhea!
 
The clucking could be because she's maturing and finding her voice more. If I pick up any of my polish they carry on with a crazy screeching like I'm killing them. Not being able to see well can cause polish to be a bit nutty. I personally trim my polish crest pretty short. I have read that some have used painters tape to put the feathers up if you don't want to trim.
 
I was ready to swallow hard this morning when I went to check on her. Still alive, still doing the exact same thing.. There were a few noticeable and rather worrying changes though. The first is her head "tick" is back, it's not necessarily a tick anymore but her head's slowly moving all over the place and it doesn't look right. The next is, the ENTIRE water dish is empty, I don't think it spilled over because it's still in the same spot I left it. Maybe she drank the whole thing, but maybe something else could've happened as well.

I think she might have ate a little bit because there were pieces of crumble at the bottom of the water dish, which is what usually happens, no noticeable changes in the food pile though, so she must not have eaten a lot. I also looked at her poop, one was green and a little runny, and the other was dark brown and runny as well. Pictures are coming. So are pics of the inside of her beak.

Unless this is a very slow moving disease it looks like the disease itself isn't gonna kill her..but the fact that she's not eating will. If this head ticking gets worse with no improvement after I try some things, I'm gonna have to cull her.

If you by ever you mean the last time she was sick, no I did not. She got completely better with only food and water. Do you think I should treat with Corid now? Her crop is completely empty, that or I can't find it. Her wattles have turned completely yellow as well. I'm REALLY concerned about her head ticking, her head is completely turned around it looks like she's trying to be an owl because her face is behind her.
