INCUBATING CHICKS DYING! HELP!!!!!!

I take eggs from the coop and put them in the incubator every day. Some are dying withing the last few days of growth and others are pipping and never coming out. I realize that I am doing it wrong since all the humidity is supposed to be adjusted per age of the egg but last year, it really went well. Most of my eggs hatched, over 100! This year, I am doing something really wrong. Would like to hear some pieces of advice....other than my huge mistake about humidity.
Actually, if your best advice just to buy another still incubator? I baby these chicks every day, turn them a touch every three hours etc. I just want em to live....

you probably have bacterial buildup. this happens when you use an incubator constantly.

if your using a Styrofoam incubator, take it apart and soak it overnight in 1 part bleach and 9 parts water. i added antibacterial soap when i used Styrofoam.

on any other incubator clean thoroughly with the same solution.

another issue on quitters could be protein. i give my roosters 18-20% during breeding season, hens get 16-18%.

do your chicks have any symptoms you can define?
 
I took your advice and cleaned it all real good. It is a Styrofoam incubator. The remainder of the chicks for the season hatched great. I was washing it all the time, but a kitchen scrub brush obviously wasn't good enough! Hopefully that was it as I prepare for another beneficial season!
