Which is most effective for treatment of lice?

Is it the same treatment time for lice as for mites (once a week for three weeks)?

Permethrin is approved for use in poultry, with no egg withdrawal period. It works very well!
Ivermectin also works fine, but is not approved for chickens in the USA, because low levels will be in the eggs for a long time. It's best not to use it!

Yes.
Unless you've got roost mites, then the coop needs to be treated too.
