I think you should stop the medications and also all that variety of food. Birds can not digest dairy products as they don't have the enzymes in their digestive system to handle them. One of the droppings look like it contains undigested meal worms.

I think she has a digestive problem - and she would be better off on a plain simple diet and plain water. I would put her in a warm place, bowl of dry layers pellets with some dry rice mixed in. And a bowl of water. Then wait for a few days to see if the droppings improve. If not, take her to a vet.

Meal worms can be hard to digest as they have a hard outer skin. If you are feeding too many or the larger variety then that will be very harmful.

Please let us know what happens.

She isn't on any medications yet (except a poultry dust for lice & mites), they are on order, and I will only be giving one at a time, starting with the wormer. The only reason I have been feeding her such a variety of food is I am trying to get her to eat something since she is so severely under weight and can hardly balance her self when I pick her up and set her down (I can feel her breast bone all the way down to her abdomen about an inch on each side). While I say I am feeding her all those foods, in reality she isn't eating them all... she only ate the scrambled eggs one time, the cooked oatmeal a few times, and the mash a few times and her crop is always empty feeling. She has been indoors in my main bathroom in the bathtub (with a towel to sit on and her food and water) to stay warm. I have had her in there since the evening of Jan 20th. I have given her a couple of warm water soaks because I thought maybe she was egg bound but nothing has passed. I have to routinely wash her fluffy but due to poop sticking to the feathers.

I'll try to get her to eat just the layer pellets with rice (is that cooked or uncooked rice?) But I'm afraid she will starve to death if she doesn't eat the meal worms. I'll admit I have been giving them to her morning and evening with some grit sprinkled on top since I don't want her to not have anything to eat. I agree about one of the pictures looking like an undigested meal worm, and I have seen them undigested in her poo previously since she has been in the bathroom. I would take her to a vet but I can afford a 100+ vet bill right now.
 
Here are the picture I promised! The first picture is from when we got her around mid November of last year and the color of the picture is the most accurate of her. The second picture is from this last week. Looking at the two pictures, I never realized how pale her comb and waddle was when I got her....perhaps she was sick before I got her, I was thinking it was something that developed since I got her! On another note, I took a fecal sample into the vet this afternoon, they said I should be hearing from them tomorrow on whether or not they find anything. The weather was nice today, around 50 degrees, so I took her out side for a bit knowing she couldn't get away from me because she can't walk super fast. She seemed to really enjoy it and scratched around a bit. She looked great (except her pale comb & distended abdomen), her tail was up and she seemed to be in good spirits.

 
Don't feed dairy products with antibiotics, not good.

Thank you for the info, I'll keep that in mind if I end up having to give antibiotics.

How is she doing? Is she eating the pellets? You can use uncooked rice no problem. My hens eat it every day. I understand you are worried about her loosing any more weight. STOP with the grit on the food. She won"t need that to digest the pellets as they are very soft when they absorb water in her body. Too much grit can also be bad for the crop.

Just an idea. Perhaps she is not eating the pellets because she is holding out for the meal worms. They actually can be addictive! Maybe you could feed chick starter crumbs mixed with the dry rice. The chick food has more energy and protein it in and would make her fatter more quickly.

She looks lovely. I am really hoping she will pull through. Do you have other hens. Sometimes having another hen with her will encourage her to eat as she sees the other one eating her food!

No I haven't heard on the results yet, I am supposed to hear sometime this morning. I was only giving her grit if I was feeding her meal worms and other foods. So she won't need grit to digest uncooked rice, and I thought uncooked rice for birds was a no no due to the rice swelling? I haven't really seen her eat much of her pellets, I was wondering if she doesn't like the pellet style and wants crumbs instead, I have seen her pick at them but just not very often. I use her pellets from her feeder I have in with her to make a mash every morning and night so it is hard to tell how much she is eating of her pellets, but she isn't eating much mash as well. Is it ok if I give her some red hard spring wheat mixed in her mash? I bought some for spouting and just haven't gotten around to it yet and she seems to like those as well as the meal worms and I can mix them in her mash easily to get her to eat the mash a little more. I'll try giving her some chick starter crumbs but I don't have any on hand yet but need to get some for the chicks we ordered that are due on 02/14! I don't really have room for another hen to put in with her and I often think that she must be lonely all day long, but really until I know what is wrong with her I would hate to have another hen exposed to her just in case.
