The thumb through the first two fingers is the ASL letter T. Shaking it back and forth, indicates that you are asking where the toilet is.

Sorry it was supposed to be a joke. I get it. My cousin is hard of hearing and in my younger days I was fairly good at ASL, I am very out of practice now however. DD is interested in learning , so I may well pick it up again.
ETA: As a nurse my sense of humor can be rather strange at times.
