Laying Hen losing weight very light

I have 1.5 year old Light Brahma hen. I treated them for coccidiosis a few weeks ago. Haven't changed anything in their diet. I treated her and the other chickens for worms the day I realized she was looking rough and felt really light. I gave them 1.6CC of liquid goat wormer. I gave her scratch grains with her pellets along with some clover and grass for some natural vitamins. she seems to be eating fine.

Any thoughts on what his could be exactly?

She seemed to get better the last few days then went down hill over night and died
